Facing the end of life can be an emotional and overwhelming journey for both individuals and their loved ones. Our hospice team is here to provide comfort, encouragement, and unwavering support during this transition. Families often experience stress, exhaustion, and uncertainty, but knowing their loved one is in the hands of compassionate and experienced caregivers offers peace of mind.

At our hospice, we create a warm, home-like environment rather than a clinical setting. We refer to those in our care as guests, honoring their dignity and ensuring they experience comfort, companionship, and quality time with family. Through personalized care, we help manage pain and symptoms, offering relief that allows guests to focus on meaningful moments with those they love.

We understand that end-of-life care extends beyond physical needs. Our hospice team provides emotional and spiritual support for both guests and their families, walking alongside them with guidance, encouragement, and a listening ear. Whether through counseling, companionship, or spiritual care, we help families find strength and peace in this sacred time.

Our services are available to all, regardless of race, gender, religious affiliation, or financial circumstances. Because hospice isn’t just about the end—it’s about making the most of every moment, with dignity and grace.

Getting Started

To begin the hospice care process, call 770-969-8354 and speak with the Director of Social Services, Director of Nursing, or the charge nurse outside of business hours, who will guide you through the evaluation process.

They will gather initial information about your loved one’s condition, explain the services offered through the Hospice Care Program, and answer any immediate questions.

Submit Health Information

Submit Health Information

A doctor’s order for hospice admission, as well as health records and other pertinent information, will be required to expedite the evaluation and ensure a smooth transition into the program.

If appropriate, we will schedule an on-site evaluation, allowing you to meet the care team and discuss your loved one’s specific needs.

Once the evaluation is complete and our services are deemed appropriate, your loved one will be admitted as a guest to the Hospice Care Program!

Qualifications & Evaluation

At SWCC, we strive to provide compassionate and professional care for our guests. Below are the necessary qualifications and the evaluation process for acceptance into our facility.

Qualifications for Acceptance

We strive to provide compassionate and professional care for our guests. To be considered for acceptance, a referral for hospice evaluation and treatment from a Licensed Care Provider is required, along with current medical records, a complete list of medications, and demographic information. If the patient is already receiving hospice care, a Certification of Terminal Illness (CTI) must also be provided.

Evaluation Process

The first step in the evaluation process is to submit a request by calling our main number and ask to speak to our Director of Social Services or Director of Nursing:

After the call, fax the necessary qualification information to: 770-969-1940.

Once we receive and review the submitted documents, our team will reach out to discuss the next steps. If appropriate, we will arrange an in-person evaluation to determine eligibility for admission.

Welcome to our Hospice Care FAQ section, where we address common questions about our compassionate services, support options, and care approaches.

What are the visiting hours?

Visitors are welcome anytime, but will need to ring the bell at the front door whenever it is locked after business hours.

Can I bring outside food in to my loved one?

Please ask the charge nurse what is appropriate for your loved one to eat. If approved, you may bring in and store outside food in the guest fridge.

What will hospice care cost us?

Our care is complimentary, but we do ask to spread the word for support as we are completely dependent on donations.

Can I spend the night with my loved one?

  • All rooms have recliners, sleeper chairs, or an additional bed in them for families to spend the night if need be.
  • We also have an application process to utilize our on-campus apartment (Hope Suites) for out-of-town guests.

Can we bring children?

All loved ones are welcome to visit our guests; however, please be considerate of other families who may be grieving.

Can we bring pets?

This decision will be based on each situation. Please ask the charge nurse about options if this is your loved one’s wish.

What are the benefits of having my loved one in an inpatient hospice setting?

  • Studies have shown that when a person enrolls in hospice care, they are more likely to have increased family satisfaction and better symptom and pain management.
  • Another special element for families to consider is that while their loved one is in our care, they can just be the family (spouse, child, sibling – whatever the relationship, even family “by love, not by blood). Wonderful, individualized care can be provided by the caring hands of our professional, interdisciplinary support team.
